The Mini Mental State Examination (MMSE) is an 11-question measure that tests five areas of cognitive function: orientation, registration, attention and calculation, recall, and language. The maximum score is 30. A score of 23 or lower is indicative of cognitive impairment. The MMSE takes only 5-10 minutes to administer and is therefore practical to use repeatedly and routinely.


El Mini Examen del Estado Mental (MMSE) es una prueba de 11 preguntas que evalúa cinco áreas de la función cognitiva: orientación, registro, atención y cálculo, recuerdo y lenguaje. La puntuación máxima es de 30. Una puntuación de 23 o menos indica deterioro cognitivo. Orientation.

Ask: "What is today's date?"

Ask for specific details such as:

  1. "What day of the week is it?"

  2. "What season is it?"

If the patient only answers with the date in numbers (e.g., "09/19/2024"), you need to ask additional questions to ensure they provide complete information. For example:

  • "Can you tell me what day of the week it is today?"

  • "What season are we in right now?"

  • Note: We need the full month name, not just the number (e.g., "September" instead of "9").

  • Reminder: Do not provide answer choices. Document the patient’s response and score it as correct (1) or incorrect (0).

Registration.

  • Read the three words to the patient at a rate of one word per second. Speak clearly and audibly.

  • The patient's score is determined by their first attempt to repeat the words. However, continue saying the words until the patient can repeat all three.

  • If the patient is unable to repeat all three words after three attempts (including the initial presentation), do not proceed to the recall test.

Attention and Calculation

  • Verbal Instructions:

    • “I would like you to spell the word ‘WORLD’ backwards.”

  • If the patient is unable to spell the word correctly:

    • Spell it Out Loud: Say, “I will spell the word for you: W-O-R-L-D. Please try to repeat it after me.”

    • Ask for Repetition: After spelling it, say, “Now, can you spell it back to me?”

  • Continue this process until:

    • The patient spells the word correctly, or

    • You have spelled it for the patient three times.

Recall

  • Verbal Instructions:

    • “Please tell me the three words I asked you to remember.”

  • If the patient has difficulty recalling:

    • Do not provide hints or prompts.

  • Document the words:

    • Record the words the patient recalls and score accordingly.

Language

  1. Naming objects:

    • Present the visual stimuli to the patient via the PowerPoint slides.

    • Verbal Instructions: “Please look at the images on the screen and name each object you see.”

  2. Repetition:

    • Verbal Instructions: “Please repeat the following phrase exactly how I say it.”

    • It is very important to speak loudly and enunciate clearly as you say this phrase. One repetition of the phrase is permissible only if it is clear that the phrase was not adequately heard.

  3. Three Step Instructions:

    • Ask the participant to place a loose sheet of paper on the desk and then take a few steps back from the computer so you can see a full view of them from the waist up. 

    • Verbal Instructions: Listen carefully. I'm going to give you some instructions. Don't start doing anything until I have stopped talking. Ready?

    • It is permissible to repeat these instructions one time if it is clear that the patient has not adequately heard the instructions. Otherwise, repetition or coaching is not allowed.

  4. Reading:

    • Display a sentence on the screen.

      • Verbal Instructions: “Please read this sentence aloud.”

      • Score correct if the participant closes his/her eyes. Reading the phrase out loud is not necessary for credit.

  5. Writing a Sentence:

    • Verbal Instructions: “Please write a complete sentence. It can be about anything.”

    • The sentence must contain a subject and verb and be sensible. Correct grammar, spelling and punctuation are not necessary.

    • Notes for Virtual Administration: Ask them to hold up their drawing to the camera for assessment and take a screenshot. Upload the image to Redcap and Box under the Patient’s folder under Screenshots.

  6. Copying a Figure

    • Verbal Instructions: “Please copy the design that I will show you.

    • All ten angles must be present and two must intersect to score one point. Tremor and rotation are ignored.

    • Notes for Virtual Administration: Ask them to hold up their drawing to the camera for assessment and take a screenshot. Upload the image to Redcap and Box under the Patient’s folder under Screenshots.

December 2023.

During a R01 Planning meeting on , the following was documented to verify all clinicians administer the MMSE the same way:

  • 1. Administer items in order.  
  • 2. Administer 3 word repetition, set timer to 3 minutes and do calculation and spelling items. To fill out the remaining 3 minutes you can administer polygons and fill it out with conversation.
  • Do not administer any other items or tasks in between. 

August 2024.

During an SLP Meeting, the following consensus was taken:

  • We decided not to count the patient's responses in a language other than the one administered in the test as correct. If the patient's results are very close to the thresholds required to be considered a candidate for the R01, the decision is made as a group whether to include them as a participant.
