- Instructor consent form.
Completion of this "Consent of Instructor" form is required prior to registration for individual instruction courses (those listed as "Member GSC") and for those courses where "consent of instructor is required." This form must be submitted to the Graduate Coordinator before attempting to register.
Completion of this form is required with a change of faculty advisor. After discussing the change with both faculty advisors, please forward the signed form to the Department.
