This policy is not intended to apply to situations where use of sick leave is appropriate.

Process

1) Before beginning, review HOP 2-2240: Modified Instructional Duties and EVPP: Modified Instructional Duties for detailed guidance and instructions.

2) Faculty member writes a request to his or her chair. See HOP 2-2240: Modified Instructional Duties, section E., "Application Requirements" for information to be included in the request.

23) The department chair should review the request following the guidance in  HOP 2-2240: Modified Instructional Duties, section F., "Application Review," and if approved, should forward the request along with an accompanying memo of support to the Dean with a cc to the Office of the Dean EA.

3) Dean then sends the Provost a memo of support along with 4) Within thirty days of receipt of the request, the dean shall submit the results of his or her review, in writing, as well as the Chair's letter and the requesting faculty's original letter.4, to the Office of the Executive Vice President and Provost. Upon final review, the Provost will communicate his or her written decision to the applicant and the appropriate dean and/or department chair. The decision of the Provost is final.

5) Provost replies to the Dean, and the Dean notifies the department of the decision whether to grant the MID.
