Upon final review, the Provost will communicate his or her written decision to the applicant and the appropriate dean and/or department chairDean's Office Faculty Affairs. to the Dean, and the Dean notifies the department of the decision whether to grant the MID6) Dean's Office Faculty Affairs will notify the department chair and Administrative Manager of the Provost's decision.

7) Department chair or AM notifies the requesting faculty member of the decision. If the MID was approved, the department should also notify the faculty member of the following in writing:

Use of available sick leave should be entered as an Absence in Workday. View the eLearning guide for Faculty to Request Sick Leave in Workday. Family Medical Leave may be used in conjunction with sick leave and should be recorded when applicable. NOTE: In accordance with HOP 5-4210, Sick leave may not be used in conjunction with Parental Leave or Family and Medical Leave once an employee has recovered from the temporary incapacity related to pregnancy. More information can be found at Handbook of Operating Procedures 5-4210.
