G.12 (1) Supplemental Materials

The Supplemental Materials folder includes twenty sections labeled (a) through (t).

In some cases, a single PDF file is requested; in other cases, a folder with multiple PDF files is needed.

The naming conventions established by the Faculty Affairs team in the Provost’s Office must be used for all files and folders in this section.

If the candidate decides not to include optional materials, or the materials do not apply to the candidate, leave that section blank, but do not renumber the section labels.
