  1. Login to the EM Customer Service Portal using the direct link above.

  2. Department: Select from drop down menu.
  3. Employee Name: Enter full name of employee.
  4. Employee EID: Enter employee EID.
  5. Employee email address: Enter employee's email address.
  6. End date: Select employee separation date from pop-up calendar.
  7. Reason for Separation: Select one.
  8. Other reason for Separation: If reason for separation was "Other", provide explanation.
  9. Is this employee transferring within the University? Select yes or no.
  10. Will you be replacing this position? Select yes or no.
  11. Does Finance and Administration need to pick up any of the following?: Select any or all choices listed.
  12. Other hardware: Enter other hardware items not mentioned above.
  13. Additional Comments/Notes: Enter additional documentation related to employee offboarding.
  14. Resignation Letter: Upload the resignation letter from exiting employee.
  15. Note: If not logged in, there will be an option to input your EID email at the bottom of the request.
  16. Click Send to submit request.
