COLA Request to Hire Form for Classified and A&P
Note: This form is required for fixed term positions and prior to recruiting for existing benefits-eligible positions regardless of the account used, and regardless if position is fixed-term or expected to continue. This form is also required for creating new benefits-eligible positions. For nonNon-benefits eligible positions , do not require the form is not required at this time. Approval from the Assistant Dean of Finance and Administration must be included in the comments of the Hire Job Requisition BP and the approval email approved Request to Hire Form must also be attached. (See Staff Recruitment page)
