COLA Request to Hire Form for Classified and A&P

Note: This form is required prior to recruiting for existing benefits-eligible positions regardless of the account used, and regardless if position is fixed-term or expected to continue. This form is also required for creating new all new or existing benefits-eligible positions. Non-benefits eligible positions do not require the form at this time. Approval from the Assistant Dean of Finance and Administration must be included in the comments of the Job Requisition BP and the approved Request to Hire Form must also be attachedThe approved form must be attached to the Job Requisition BP.
