- CNS FWA Request Proposal Form for 3 or more days remote weekly (additional approvals required)CNS FWA Request Proposal Form for – Chair/Dept Head approval required (as the optional signer)
- 100% remote work outside of Texas (dean's approval required)- Chair/Department Head approval required (as the optional signer) in addition to Dean’s approval. Please forward completed form to CNS HR at personnel@cns.utexas.edu for review by the Dean.
