For 2 or fewer days remote weekly – supervisor approval required
3 or more days remote weekly – Chair/Dept Head approval required (as the optional signer)
100% remote work outside of Texas - Chair/Department Head approval required (as the optional signer) in addition to Dean’s approval. Please forward completed form to CNS HR at personnel@cns.utexas.edu for review by the Dean.
