All student comments from formal, end-of-semester course evaluations (CIS and CES) collected during the time window defined in Subsection Section G.6(c) must be placed in a separate folder entitled “Student Comments” within the Supplemental Materials folderfolder in this section.

The comments related to each class (defined by unique number) must be saved as a separate PDF document in the Student Comments folder. Courses , and courses must be organized in chronological order and within the folder.

The CIS summary evaluation page or CES summary must be included as the first page of the PDF each document, followed by the complete set of student comments for the relevant course.
