All student comments from formal, end-of-semester course evaluations (CIS and CES) collected during the time window defined in Subsection G.6(c) must be placed in a separate folder entitled “Student Comments” within the Supplemental Materials folder.

The comments related to each class unique number must be saved as a separate PDF document in the Student Comments folder.

Courses must be organized in chronological order and the CIS summary evaluation page or CES summary must be included as the first page of the PDF followed by the set of student comments for the relevant course.
