Cooperating teachers in AISD and RRISD must submit an application to the district in order to host a student in their classroom. The application must be signed by the principal and received by the district in order for the placement to be approved.
Coordinators will supply teachers in Austin ISD with Cooperating Teacher Applications each semester. The applications are completed electronically through DocuSign. Coordinators will be given the links to the DocuSign forms to share with their CTs each semester. Below are the links for fall 2023:

Additionally, CT applications will be required for all cooperating teachers, not just those in Austin ISD. Below are generic Cooperating Teacher Agreement forms for you to use when making placements in other districts, charter/private schools, etc. Note: Round Rock ISD will distribute their own Agreement forms to their teachers. Pflugerville ISD also has their own placement procedure. They have asked that we not distribute forms to their teachers until the district has approved the placement.
