An image of the final prototype of the mechanism can be seen below in Figure 1.

The final prototype of the mechanism worked as intended and was able to convert rotary motion into linear motion by moving the slider up and down through the rotation of the input link. This mechanism was made with 1/4" plywood, 1/4" bolts, nuts, and washers, and an 18 in long undermount drawer slides. The video below shows the mechanism moving through a full 360-degree revolution.
