These forms are completed in DocuSign and will route directly for signatures. As we transition to DocuSign templates, we would appreciate any feedback on the forms themselves. Once your form has been final approved, simply attach the signed PDF version (document and certificate combined) to the Workday process you are submitting.  
 
When using these forms, please be sure to use your EID email address so you can find your form later. An example EID address would be: abc123@eid.utexas.edu.

Note: This form is required for fixed term positions and benefits-eligible positions regardless of the account used. This form is also required for creating new positions. For non-benefits eligible positions, the form is not required at this time. Approval from the Assistant Dean of Finance and Administration must be included in the comments of the Hire BP and the approval email must also be attached.
