- Login to the EM Customer Service Portal using the direct link above.
Remember to login using your EID email address YOUREID@eid.utexas.edu.
For more information on logging in go to EM Jira Service Management - Department: Select from drop down menu.
- Employee Name: Enter full name of employee.
- Employee EID: Enter employee EID.
- Employee email address: Enter employee's email address.
- End date: Select employee separation date from pop-up calendar.
- Reason for Separation: Select one.
- Other reason for Separation: If reason for separation was "Other", provide explanation.
- Is this employee transferring within the University? Select yes or no.
- Will you be replacing this position? Select yes or no.
- Does Finance and Administration need to pick up any of the following?: Select any or all choices listed.
- Other hardware: Enter other hardware items not mentioned above.
- Additional Comments/Notes: Enter additional documentation related to employee offboarding.
- AttachmentResignation Letter: Upload the resignation letter from exiting employee.
- Note: If not logged in, there will be an option to input your EID email at the bottom of the request.
- Click Send to submit request.
