Transcript Authorization Forms
As of AY 2020-21, Transcript Authorization Forms are required only when Faculty Credentialing requests them. EVPP offer letter templates include a statement that offers are contingent upon satisfactory completion of credentialing. Strategic Academic Initiatives oversees the transcript authorization process and will reach out to colleges or departments if a form is needed.
New faculty hires will receive an onboarding task in Workday asking them to upload their official transcript for their highest degree earned. Once this step is completed, APS will initiate the credentialing process.
If an academic department wishes to proceed with credentialing prior to acceptance of an offer letter by a prospective faculty hire, they should complete the Official Transcript Authorization for New Faculty form and email to faculty.credentialing@utexas.edu. Include in the email a list of courses the faculty member is expected to teach.
If the degree was earned from an international institution, the faculty member should also provide Strategic Academic Initiatives a copy of their official transcript from the institution or official letter of degree verification that includes the faculty member's name, name of degree and major, and date of award.
